Today, I have Dr. Sue Yanoff here with me to talk about sports medicine and what to do when something goes wrong with our sports dogs. Hi, Sue. Welcome back to the podcast. Hi, Melissa. It's good to be back. I'm excited to have you. It's been a while. It has been a while. To start us out, you want to remind listeners a little bit about you and your current furry crew? Yes. I think the most important thing to know is that I am a veterinarian and I am a board certified surgeon. I have two beagles. I compete in agility and obedience and I have done tracking. So I have Ivy, who's 13 and retired. She's a champion, has her mock CDX, RE and TD. And Quinn, the younger one, who's almost five now, is working on her MX, MXJ. And we will start trialing and rally and obedience sometime this summer. Exciting. Yeah. Good stuff. All right. So I have a ton that I'm hoping to cover in our conversation today. So fingers crossed we get through it all. But to start us out, I think it makes sense to talk about the types of injuries that we tend to see in sports dogs. So obviously, you know, there are the issues that are genetic. But if we set those aside, what types of injuries arise because of the games that we play with our dogs? The most common injuries are soft tissue injuries. So it is possible for a dog to sustain a fracture doing sports, but that's not very common. And there was an Internet survey that was published a few years back that talked about the most common injuries. And while Internet surveys are not the best science, it is a good way to review what is going on. And it matches what I've seen in practice. So the most common soft tissue injuries that I see are shoulder injuries, specifically supraspinatus tendonitis, bicep tendonitis, and medial shoulder instability. And then we see a lot of dogs with ileozoic muscle strains. And then I also see a lot of toe injuries. Some of them are sprains. I have seen a fracture or two of the toe doing agility. So I would say those are the three most common injuries. Gotcha. It's not super uncommon, I guess, to kind of hear that somebody, you know, they were training or they were competing and their dog kind of came up limping, you know, after they walked off the field. Does that necessarily mean that it's something serious?

아니면 개들도 사람이 발목을 삐는 것과 비슷한 경험을 할 수 있나요? 그렇다면 어떻게 판단해야 할까요? 네, 반드시 심각한 것은 아닙니다. 그리고 염좌는 심각한 부상입니다. 염좌는 인대 부상을 말합니다. 그래서 개들이 심각하지 않은 여러 이유로 다리를 절며 나오는 경우를 자주 보게 된다고 생각합니다. 2주 전 어질리티 대회에 갔을 때 한 개가 점프대에 부딪힌 후 뒷다리를 절기 시작했습니다. 하지만 그 개가 경기장 밖으로 나왔을 때는 더 이상 다리를 절지 않았습니다. 또 다른 개는 달리기 후반부에 다리를 절었습니다. 주인이 개를 데리고 나왔는데 확인해보니 발바닥 패드에 찰과상을 입은 상태였습니다. 따라서 부상이 반드시 심각한 것은 아닙니다. 사람도 걷다가 발가락을 찧으면 매우 아프잖아요. 하지만 몇 분 지나면 통증이 사라지죠. 그래서 더 이상 다리를 절지 않게 되는 겁니다. 아니면 발목을 옆으로 삐끗해서 많이 아플 수도 있죠. 하지만 연부 조직에 심각한 손상은 없습니다. 그래서 몇 분이나 한 시간 정도 지나면 더 이상 아프지 않게 됩니다. 그러니 네, 반려견이 스포츠 활동을 하다가 부상을 입을 가능성은 있습니다. 그리고 그게 반드시 심각한 부상은 아닐 수도 있고요. 하룻밤이 지나도 나아지지 않는다면 진찰을 받아봐야 한다고 생각합니다. 잠시 그 점에 대해 생각해 보면, 사실 우리 반려견들은 통증을 숨기는 데 정말 능숙합니다. 그리고 바라건대, 스포츠를 즐기는 반려인으로서 우리는 일반적인 반려인들보다 반려견과 조금 더 교감이 잘 된다고 생각합니다. 하지만 제 질문은 두 가지 측면이 있습니다. 우리가 주의해야 할 미묘한 징후들은 어떤 것이 있을까요? 반려견이 여전히 아픈 상태인데도 통증을 잘 숨기고 있다는 것을 알려주는 어떤 신호 말이죠. 아니면, 동물 병원에서 과장하는 것으로 치부되거나 '더 이상 다리를 절지 않으니 괜찮을 거다'라는 식으로 무시당하지 않게 정보를 잘 전달하려면 어떻게 해야 할까요? 그러니까 괜찮겠지, 하는 그런 반응들요. 그런 거 있잖아요. 맞아요. 좋은 질문입니다. 우리는 일반 반려견에게서는 알아차리지 못할 스포츠 독의 미묘한 변화를 자주 목격한다고 생각합니다. 반려견이 다리를 절거나 다리를 들고 있다면 그건 명확하죠. 하지만 특히 어질리티를 하는 개들에게서 제가 자주 보는 흔한 징후 중 하나는 기록이 평소보다 조금 느려지는 경우입니다. 반려견이 회전 구간을 평소보다 넓게 돌거나 바를 건드리는 경우죠. 평소 바를 건드리지 않던 개가 건드리거나, 점프 직전에 주춤거리다가 뛰는 경우도 해당합니다. 위브 폴을 하다가 중간에 빠지는 경우나 다른 수행 능력의 변화도 흔히 나타나는 증상입니다. 그래서 그런 것들이 보통 선수들이 알아차리는 징후들이죠.
Or is it possible for dogs to kind of have the equivalent of spraining an ankle? And then, you know, how do we tell? Yeah, it's not necessarily serious. And a sprain is a serious injury. A sprain is an injury to a ligament. So I think often we'll see dogs limp off for various reasons that are not serious. I was at an agility trial two weekends ago and a dog crashed into a jump and started limping on the hind leg. By the time the dog came off the arena and out of the ring, he wasn't limping anymore. And then another dog was limping towards the end of her run. The owner brought her out and she ended up having an abrasion of one of the pads. So injuries are not necessarily serious. Like we can be walking around and stub our toe and it hurts a lot. But then after a few minutes, it doesn't hurt anymore. So you're not limping anymore. Or we can like twist our ankle like to the side and it hurts a lot. But there's no serious damage to any soft tissue. So after a couple of minutes or maybe an hour or so, it doesn't hurt anymore. So yes, it is possible for a dog to injure themselves doing their sport. And it can be not necessarily serious. I would say if it doesn't get better overnight, then I would say it needs to be checked out. Thinking about that for a minute, you know, often our dogs are so good at hiding pain. And, you know, hopefully as sports antlers, we're a little bit more in tune with their dog than maybe the typical pet owner. But I guess my question is kind of twofold. I guess what are some of the more subtle signs that we need to watch out for? Something that says, you know, maybe the dog's still hurting, but they're doing a good job of hiding that pain. Or and how do we kind of present that information so that we're taken seriously at the vet and it's not maybe dismissed as an exaggeration or, you know, the dog's not limping anymore. They must be fine. You know, that kind of thing. Right. Those are good questions. I think we often see subtle changes in sports dogs that we would not notice in a pet dog. So if a dog is limping or holding a leg up, that's obvious. But some of the more common things I see, especially in agility dogs, are things like the times are a little bit slower. The dog is taking wide turns, wider than normal or knocking bars. The dog that doesn't normally knock bars is knocking bars or stuttering up to the jump and then jumping. A common thing is popping the weaves or other changes in performance. So those are usually the things that a competitor notices.

훈련 중에 강사가 '저 개는 뭔가 좀 이상하다'라고 해서 저에게 데려온 개들도 본 적이 있습니다. 그리고 제가 그 개를 진찰해 보니 부상을 발견한 경우도 있었죠. 또한 도그쇼에 출전하던 개 몇 마리를 본 적도 있습니다. 그들의 이야기는 '개를 스택 자세로 세우면 항상 다리 한쪽을 살짝 움직인다'는 것이었죠. 예전에 본 코기 한 마리가 기억나는데, 보호자의 유일한 불만은 쇼 링에서 스택 자세를 취하게 하면 오른쪽 뒷다리를 0.5인치 정도 앞으로 계속 움직인다는 것이었습니다. 그래서 검사해 보니 꽤 심각한 장요근 염좌를 앓고 있었습니다. 그러니 증상이 미미하다고 해서 심각한 부상이 아니라는 뜻은 아닙니다. 즉 임상 증상이 항상 부상의 심각도와 일치하는 것은 아니라는 거죠. 물론 우리 모두 알고 있듯이, 의욕이 넘치는 퍼포먼스견들은 어질리티를 하거나 사냥 혹은 다른 스포츠에 참여할 때는 부상을 숨기고 아픈 내색을 하지 않습니다. 그러다 집에 돌아와 잠을 자고 일어났을 때 몸이 뻣뻣해지면 비로소 문제가 있다는 것을 알아차리게 되죠. 하지만 사실 종종 그 이력은 퍼포먼스 저하로 나타납니다. 스포츠에 대해 잘 모르는 일반 수의사는 '우리 개는 잘하고 있는데 위브 폴(weaves)을 자꾸 빠져나와요, 뭔가 잘못된 걸 알겠어요'라고 말하면 이상하게 쳐다볼 겁니다. 또한 정형외과적 검사에 대한 경험이나 훈련이 부족한 일반 수의사는 어디를 봐야 할지, 어떻게 검사해야 할지 모르기 때문에 이상을 발견하지 못할 것입니다. 그래서 이 부분은 나중에 더 자세히 이야기하겠지만, 일반 수의사가 시도해 볼 만한 적절한 치료법을 권했음에도 반려견이 나아지지 않는다면 우리는 진단을 위해 다른 곳을 찾아보기 시작해야 합니다. 일반 동물병원 진료를 받으러 갈 때 추천하고 싶은 사항 같은 게 있을까요? 그러니까 동물병원에 갈 정도로 심각한 상황이라고 판단하게 만든 무언가가 분명히 있었다고 가정하는 거죠. 저희가 진료 시 증상을 심각하게 받아들이게 하거나, 가능하다면 정확한 진단을 받을 수 있는 확률을 높이기 위해 할 수 있는 일이 있을까요? 긍정적인 결과를 얻을 확률을 높이기 위해, 저희가 가장 잘 준비할 수 있는 방법 같은 게 있을까요? 네. 가장 좋은 방법은 수의사분께 반려견 스포츠에 대한 속성 교육을 해드리는 것입니다. 예를 들어 어질리티를 한다면, 절뚝거리지는 않더라도 퍼포먼스가 저하되는 경우 반려견에게 문제가 있을 수 있다는 점을 설명할 수 있죠.
I've also seen dogs that were brought to me because during a lesson, the instructor said, you know, there's something off with that dog. And then I've seen the dog and we have found injuries. And then I've also seen a couple of dogs that were being shown in breed. And the history is when I stack the dog, he always moves one leg slightly. So I remember a corgi we had once that the only complaint was that when they stacked him for the show ring, he moved his right hind leg forward just like half an inch and consistently. And then we checked him and he ended up having a pretty severe iliopsoas muscle strain. So, you know, just because the signs are subtle doesn't mean that it's not a serious injury. So the clinical signs don't always match with the severity of the injury. And of course, we all know that our high drive performance dogs will hide their injury and not worry about it while they're running agility or doing their hunting or whatever other sport they're engaged in. And then when they get home and sleep on it and then they get up and they're stiff, then we notice that there might be a problem. But oftentimes, really, the history is a performance issue. And a general practitioner who doesn't know much about sports would look at you funny if you said, oh, my dog's doing great, but he's popping the weaves and I know something's wrong. And then a general practitioner who is not necessarily experienced or trained in doing orthopedic exams is not going to find anything wrong because either they don't know where to look or they don't know how to look. So and we'll talk about that later. But if your general practitioner recommends a treatment, which is perfectly reasonable to try and your dog doesn't get better, then we have to start looking elsewhere for a diagnosis. Do you have, you know, kind of anything that you'd recommend going into that appointment with that general practitioner vet? You know, assuming something obviously came up that made you consider it serious enough to warn a vet visit. Is there anything we can do, I guess, to kind of increase the likelihood that we do get something kind of taken seriously, that we do get maybe a diagnosis if possible? You know, is there anything we can do to kind of be our best prepared, most prepared selves to increase the likelihood of things, you know, having a positive outcome? Yeah. So probably the best thing you do is give your vet a crash course in sports. So if you do agility, you can explain to the vet that oftentimes dogs that are not lame but are having a decrease in performance have a problem.

하지만 할 수 있는 일은 그리 많지 않습니다. 수의사는 할 수 있는 최선의 검사를 진행할 테니까요. 수의사는 흔히 비스테로이드성 소염진통제(NSAIDs) 처방과 활동 제한 같은 보존적 치료를 권장하는데, 이는 매우 합리적인 조치입니다. 만약 심각한 문제라면 수의사가 그것을 놓칠 리가 없습니다. 만약 증상이 미묘하다면 수의사가 놓칠 수도 있습니다. 그리고 그런 미묘한 문제가 제대로 치료되지 않으면 심각해질 수 있죠. 그래서 저는 대부분의 경우, 처음에 보존적으로 반려견을 치료하는 것에 대해 딱히 거부감이 없습니다. 하지만 2~3주가 지나도 반려견의 상태가 나아지지 않는다면, 그때는 더 자세히 살펴봐야 한다고 생각합니다. 그러니까 2~3주라는 기간이 핵심적인 요소인 것 같네요. 더 정밀한 진단을 받아봐야 한다는 신호로 볼 수 있는, 강조하고 싶은 다른 위험 신호들이 있을까요? 음, 일반 동물병원에 다녀온 뒤에도 반려견의 상태가 더 나빠진다면, 그때는 2~3주를 기다리면 안 됩니다. 하지만 일반적으로 반려견의 상태가 악화되지 않고, 다리를 들고 걷는 것과 같은 심각한 파행이 아니라면 괜찮습니다. 다리를 들고 걷는 증상은 즉각적인 조치가 필요합니다. 그러니 수의사가 '아무것도 발견하지 못했으니 며칠 더 지켜보자'고 해도 저는 괜찮다고 생각합니다. 그 정도면 괜찮다고 생각해요. 얼마나 더 깊게 파고들고 싶은지에 따라 다르겠죠. 하지만 1~2주 안에 나아지지 않거나 오히려 더 나빠진다면, 더 자세히 알아봐야 합니다. 알겠습니다. '더 자세히 알아본다'는 게 무슨 의미인지 이야기해 보죠. 그러니까 어떤 식으로 더 자세히 알아봐야 하는지, 무엇을 찾아야 하는지에 대해 어떻게 권장하시는지 말씀해 주세요. 수의사에게 받은 답변이 불확실하거나 문제가 해결되지 않았을 때, 핸들러가 어떻게 더 적극적으로 진단을 요구할 수 있을까요? 네, 제 생각에 핵심은 진단을 위해 어디를 더 찾아봐야 하는지 아는 것입니다. 진단을 받는 것은 매우 중요하다고 생각합니다. 개가 처음 다쳤을 때 일반 동물병원에 데려갔는데 별다른 이상을 찾지 못하고 그냥 연부 조직 손상일 것 같다고 말하는 경우 말이죠. 음, '그냥'이라는 표현은 쓰고 싶지 않네요. 일부 연부 조직 손상은 매우 심각할 수 있으니까요. 하지만 경미한 염좌(인대 손상)나 경미한 좌상일 가능성도 있습니다. 좌상은 근육 손상을 말합니다. 1~2주 정도 치료하면 나아질 겁니다. 그러면 개는 다시 정상으로 돌아오겠죠. 하지만 나아지지 않거나 핸들러가 '수의사는 아무것도 못 찾았지만, 우리 개 상태가 뭔가 이상해요'라고 느낀다면요. 더 자세히 알아보기 위해 가야 할 곳은 두 군데입니다.
But there's not much you can do. The vet is going to do an exam as best they can. They often will recommend conservative treatment consisting of often NSAIDs and restricted activity, which is very reasonable. If there's if it's nothing, if it's something serious, the vet is not going to miss it. If it's subtle, the vet might miss it. And that subtle problem could become serious if it's not treated properly. So in most cases, I don't have a problem treating a dog conservatively at first. But then after two or three weeks, if your dog's not getting better, then I think we have to look further. So two or three weeks seems like the key factor there. Are there any other things that maybe you'd highlight as red flags that mean we should, you know, pursue a more serious diagnosis? Well, if the dog is getting worse after you see your regular vet, then yes, you don't want to wait two or three weeks. But in general, if the dog's not getting worse and it's not a serious lameness, like if the dog's walking around with one leg held up, that's serious. So that has to be taken care of right away. So I'm fine if the vet says, I can't find anything. Let's just see how he is in a few days. That's kind of okay, I guess. It depends on how far you want to pursue things. But if it doesn't get better within a week or two or if it gets worse, then we need to look further. Okay. So let's talk about what that means when you say look further. So talk to me about how you recommend kind of what is looking for, what does looking for there look like, I guess. How can a handler, you know, push more for a diagnosis if, you know, the answers they're getting from their vet are less definitive or, you know, didn't solve the issue? Okay. So that's the key, in my opinion, is to know where to look further for a diagnosis. And I think it's very important to get a diagnosis. When the dog is first injured and you go to your general practitioner and they can't really find anything and they say, well, it's probably just a soft tissue injury. Well, I don't like to say just because some soft tissue injuries are very serious. But it is possible it's a mild sprain or a sprain is an injury to a ligament or a mild strain. A strain is an injury to the muscle. And with a week or two of treatment, it'll get better. And then you get the dog back to normal. But if it doesn't get better or the handler says, you know what, my vet can't find anything, but there's something not right with this dog. There's two places to go to look further.

미국 수의외과학회(ACVS)와 미국 수의스포츠의학 및 재활학회(ACVSMR)입니다. 정형외과 진료를 많이 보는 외과 전문의를 찾아가야 합니다. 일부 전문의 자격을 갖춘 정형외과 수의사들은 스포츠견에 대한 경험이 매우 많습니다. 어떤 수의사들은 외과와 스포츠 의학 자격을 모두 갖추기도 했습니다. 그리고 미국 수의스포츠의학 및 재활학회 소속 수의사들은 스포츠 의학과 재활을 전문적으로 훈련받은 전문가들입니다. 물론 그들이 수술 교육을 받는 것은 아니지만, 훌륭한 진찰을 수행하고 진단을 내리기 위해 반드시 수술 교육이 필요한 것은 아닙니다. 따라서 만약 다니는 수의사에게 의뢰할 만한 사람이 없거나, 보드 인증 외과 전문의나 스포츠 의학 수의사를 모르는 경우라면, 해당 협회 웹사이트에 접속해서 거주 지역의 전문의나 보드 인증 수의사를 찾아볼 수 있습니다. 저도 제 수업을 듣는 학생들을 위해 자주 그렇게 합니다. 학생들에게는 진단이 필요하고 전문가 진료가 필요한데, 누구에게 가야 할지 모르는 경우가 많거든요. 그래서 학생들이 사는 곳을 물어본 뒤, 제가 웹사이트에 접속해 해당 지역의 전문가를 찾고 그들의 이력이나 관심 분야, 경험 등을 확인합니다. 그런 다음 그 정보를 바탕으로 누구를 만나야 할지 추천해 줄 수 있죠. 확실한 진단을 받는 것이 항상 가능한가요? 아니요, 제가 웃는 이유는 제 개가 그에 대한 좋은 예이기 때문입니다. 하지만 대다수의 경우에는 우리가 '확정 진단'이라고 부르는 진단을 받을 수 있다고 말씀드리고 싶습니다. 즉, 무엇이 문제인지 확실하게 알 수 있는 진단이죠. 검사 결과 어깨 통증이 확인되면 그 원인이 될 수 있는 질환 목록을 알려드릴 수는 있습니다. 하지만 추가 검사를 하지 않고서는 그중 무엇이 통증의 원인인지 확정적으로 말할 수 없습니다. 그리고 많은 경우 추가 검사가 꼭 필요한 것은 아닙니다. 만약 개가 2주 정도 휴식하고 소염진통제를 복용해서 나아진다면, 굳이 추가 검사를 할 필요는 없죠. 하지만 확정 진단을 원하거나, 일반적인 보존적 치료로도 개의 상태가 호전되지 않는다면, 그때는 더 정밀한 검사가 필요합니다. 방사선 촬영이 필요할 수 있습니다. 초음파 검사, 즉 근골격계 초음파 검사가 필요할 수도 있죠. 물론 이 모든 것은 아주 철저한 정형외과적 검사에서 시작됩니다. 그리고 몇몇 전문가들은 보행 분석판(force plate) 검사를 활용하기도 합니다. 개를 보행 분석판 위로 걷게 하면 어느 한쪽 다리에 체중을 싣지 않는지 확인할 수 있죠. 다른 다리들에 비해 특정 다리에 체중을 덜 싣고 있다는 것을 알 수 있습니다.
And these are the American College of Vietnary Surgeons and the American College of Vietnary Sports Medicine and Rehabilitation. So you want to see either a surgical specialist and that somebody that does a lot of orthopedics. And some board certified orthopedic surgeons have a lot of experience with sports dogs. Some surgeons also are dual qualified in surgery and sports medicine. And the American College of Vietnary Sports Medicine and Rehabilitation are veterinarians that are trained in sports medicine and rehabilitation. Obviously, they don't get training in surgery, but they don't necessarily need training in surgery to do a good exam and make a diagnosis. So if your veterinarian does not have somebody that they can refer you to, if they don't know a board certified surgeon or a board certified sports medicine vet, then you can get on the website of either of those and look up a diplomat or a board certified veterinarian in your area. And that's what I do a lot for the students in my class. They I need them to get a diagnosis and I need them to see a specialist, but they don't know who to go to. So they tell me where they live. Then I get on the website and I look up specialists in their area and then I read the bio of that specialist and see what their interests are and what their experience is. And then based on that information, I can make a recommendation of who they should see. Is it always possible to get a firm diagnosis? No, which I'm laughing because my dog is a good example of that. But let me say that in the vast majority of cases, yes, you can get what we call a definitive diagnosis, which is a diagnosis that we should we're sure that's what the problem is. When we do an exam and we get shoulder pain, I can give you a list of things that it might be. But I don't know definitively which one of those is causing the pain unless I do further tests. And a lot of times it's not necessary to do further tests. If the dog gets better with a couple of weeks rest and NSAIDs, we don't need to do further tests. But if you want a definitive diagnosis, if your dog is not getting better with just standard conservative treatment, then yes, you need to go further. You need radiographs. You might need an ultrasound exam, a musculoskeletal ultrasound exam. And then and of course, it all starts with a really thorough orthopedic exam. And some some practitioners have force plate analysis. So you trot your dog over a force plate and you can tell if one leg is is not putting it. The dog's not putting as much weight on one leg as the others.

그 외에도 MRI나 CT, 혹은 관절 내부를 직접 들여다보는 관절경 검사처럼 더 정밀한 진단 검사들도 있습니다. 하지만 제 어린 비글도 오른쪽 뒷다리를 깡충거리는데, 한 살도 안 됐을 때부터 그랬어요. 슬개골 탈구(medial patella luxation)가 있는 개들이 그러하듯 가끔 다리를 절뚝거리며 걷기도 하고요. 그래서 제가 직접 검진해 봤지만 아무것도 찾을 수 없었습니다. 스포츠 재활 전문 병원에 데려가 검진을 받았을 때도 그 다리에 체중을 덜 싣고 있는 상태였죠. 방사선 검사를 했지만 정상으로 나왔습니다. 초음파 검사에서는 이상 소견이 발견되어 약 6개월간 치료를 진행했습니다. 그런데 다시 정상적인 활동을 시작하자마자 또다시 다리를 절뚝거리기 시작하더군요. 그래서 다음 단계로는 요추 MRI 검사를 해야 하는 상황이었습니다. 하지만 임상 증상이 너무 경미하고 개도 크게 불편해하지 않아서 검사를 거절했습니다. 임상 증상이 그렇게 경미한데 척추 수술까지 시키고 싶지는 않았거든요. 그래서 그냥 다리를 절뚝거리며 지내게 두고 있는데, 개는 전혀 불편해하지 않고 활동성에도 영향이 없습니다. 아마 제 개보다는 제가 더 신경 쓰이는 것 같네요. 네, 어쨌든 제 개는 이상이 있는 사례입니다. 개가 다리를 저는 것은 정상적인 행동이 아니니까요. 사람들이 흔히 비글이나 잭 러셀 테리어는 가끔 다리를 저을 수도 있다고 말하지만요. 원래 그런 거라고들 하지만, 사실 정상은 아닙니다. 하지만 왜 그런 행동을 하는지는 알 수 없는 상태입니다. 증상이 더 악화되지 않는다면 끝내 그 원인을 모를 수도 있겠죠. 더 이상의 정밀 검사는 진행하지 않을 생각입니다. 그래도 이미 충분히 많은 진단 검사를 해보았으니까요. 그래서 만약 상태가 더 나빠진다면 확실히 MRI 촬영을 할 거예요. 그리고 그게 우리가 답을 얻는 데 도움이 되기를 바라야죠. 하지만 그러지 못할 수도 있습니다. 그러니 다리를 절거나 건너뛰는 증상으로 문제가 생기지 않기를 간절히 바라야겠네요. 네. 네. 그러게요, 네. 하지만 대부분의 경우 우리는 진단을 내릴 수 있습니다. 대부분은 가능하죠. 하지만 가장 가능성이 높은 원인이 무엇인지에 따라 치료 옵션이 무엇인지 고려해 볼 가치가 있는 것 같아요. 가장 가능성 높은 원인이 무엇이라고 생각하는지에 따라서요. 어쩌면 징후나 증상에 비춰볼 때 말이 안 될 수도 있죠. 맞아요. 그러니까, 문제가 있는 걸 발견하더라도 그게 경미할 수도 있잖아요. 그리고 수술로 치료할 수 있다는 걸 알고 있는 질환일 수도 있고요. 하지만 수술에는 항상 위험이 따릅니다. 그래서 항상 위험과 이득을 비교해서 따져봐야 합니다. 별로 심각하지 않고 강아지를 괴롭히지도 않으며 강아지가 평생 안고 갈 수 있는 정도라면요. 그럼 저는 수술하지 말라고 할 거예요. 슬개골 탈구가 완벽한 예시죠.
And then there are more advanced diagnostic tests like an MRI or a CT or even arthroscopy to look into the joint. But yeah, my my younger beagle hops on her right hind leg and she's been doing that since she was less than a year old. So she skips on it sometimes like you would expect a dog with a medial patella luxation to do. So I examined her and couldn't find anything. And so I brought her to a sports medicine practice and they examined her and she was taking some weight off that leg. And they did radiographs, which were normal. And they did an ultrasound exam, which found an abnormality, which we treated for about six months. And then as soon as I started getting her back to normal activity, she started skipping again. So the next step would have been to do an MRI of her lumbar spine. But I declined that because her clinical signs were so mild and they didn't bother the dog. And I was not about to do spine surgery in a dog that had such mild clinical signs. So she basically lives with skipping and it doesn't bother her and it doesn't affect her performance. It probably bothers me more than it bothers her. So, yes, she's an example of a dog that has an abnormality because it's not normal for dogs to skip, even though people often say, oh, you know, sometimes beagles skip or sometimes Jack Russell Terry is skipped. They just do it. But it's it's not normal. But we don't know why she's doing it. And we might never know why if it never gets worse. I'm not going to pursue any further diagnostics. But we did do a lot of diagnostics. So if she was getting worse, then I for sure would do an MRI. And that hopefully would give us an answer. But it might not. So fingers crossed that she won't have any problems with her skipping. Yeah. Yeah. So, yeah. But most of the time we we can get a diagnosis. Most of the time we can, but it sounds like it's worth kind of thinking through what are the treatment options, depending on what we think are the most likely causes. And maybe it doesn't make sense based on signs, symptoms. Right. I mean, you know, we could we could find something wrong and it could be mild. And it's a condition that we know can be treated with surgery. But surgery is not without risks. So you have to always weigh the risk versus a benefit. So if it's something that's not very serious and it's not bothering the dog and it's something the dog can live with. Then I would say just don't do surgery. Patella luxation is a perfect example.

강아지가 1단계 슬개골 탈구가 있어서 가끔 다리를 절더라도, 보호자가 원하는 민첩성 훈련이나 패스트 캣, 복종 훈련 등 무엇이든 할 수 있다면요. 그럼 저는 절대 그 강아지에게 수술을 권하지 않을 겁니다. 하지만 2단계나 3단계 혹은 그보다 심한 슬개골 탈구로 다리를 자주 절고, 강아지가 스포츠 활동을 하는 데 지장을 준다면 저는 찬성할 거예요. 그때는 수술로 얻는 이득이 위험보다 크기 때문에 수술을 고려해야 합니다. 알겠습니다. 알겠어요. 운동 제한 같은 조치가 권장되는 경우, 이와 관련해 의료적 지원 측면에서 어느 정도까지 요구하는 것이 합리적인지 조금 말씀해 주실 수 있나요? 수의사들은 흔히 활동 제한을 권장하지만, 에너지가 넘치는 반려견에게 그것이 얼마나 어려운 일인지 미처 생각하지 못할 때가 많은 것 같아요. 휴이가 발톱이 뽑혔을 때, 수의사 선생님이 '활동을 못 하게 하세요'라고 하더군요. 그래서 저는 '얘는 바닥에 피를 흘리고 있어도 상관 안 하는 애예요'라고 말했죠. 그건 얘한테 전혀 문제가 안 되거든요. 그래서 약을 처방해 달라고 요청해야 했어요. 아이를 조금이라도 진정시킬 수 있는 무언가가 필요했으니까요. 그 부분에 대해 좀 더 자세히 이야기해 주실 수 있나요? 네. 저도 퀸과 6개월 정도 그 과정을 겪었는데, 그동안 허용된 유일한 운동은 리드줄을 매고 걷는 것뿐이었어요. 물론 다른 정신적 자극이나 기본적인 훈련, 신체에 무리가 가지 않는 훈련은 했지만, 달리기나 점프는 절대 금지였죠. 제가 말하는 활동 제한은 크레이트(케이지)에만 가둬두는 것을 의미하지 않습니다. 배변 산책을 제외하고는 개를 크레이트나 울타리(X-pen) 안에만 두도록 하는 아주 엄격한 크레이트 휴식을 권장하는 경우는 딱 두 가지뿐이에요. 하나는 수술이 불가능하거나 비용 문제로 수술을 할 수 없는 골반 골절의 경우이고, 다른 하나는 디스크 질환으로 극심한 통증이나 신경학적 결함이 있는 경우입니다. 그런데 어떤 이유로든 개가 수술을 받지 않는 상황일 때죠. 그 두 가지 경우를 제외하고 제가 진료하는 대부분의 연부 조직 부상에서, 저에게 활동 제한이란 달리기 금지, 점프 금지, 훈련 금지, 다른 개와 놀기 금지, 계단 오르내리기 금지를 뜻합니다. 보호자들에게 반려견이 침실에서 함께 잔다면 밤에 천천히 계단을 올라가게 하거나 아침에 내려오게 하는 정도는 괜찮다고 말하죠. 리드줄을 매고 5~10분 정도 하루에 여러 번 나가서 냄새를 맡게 하는 것은 괜찮지만, 뛰거나 속보로 걷는 것은 최소화하거나 금지해야 합니다.
If a dog has a grade one patella luxation and skips on that leg occasionally, but can do anything the owner wants to do, agility or fast cat or any other or obedience, whatever they want to do. Then I would absolutely say do not do surgery on that dog. But if the dog has a grade two or three or worse patella luxation and it's hopping more often than not and it's affecting how the dog is is doing her sport, then I would say yes. Then the risk of the benefit of surgery outweighs the risk and that dog should be considered for surgery. Gotcha. Gotcha. So in those cases where something like restricted movement is recommended, right, can you talk a little bit about what it's reasonable to ask for in terms of like medical assistance with that? So I think a lot of times vets recommend restricted activity and sometimes they don't necessarily think about how difficult that might be with a higher drive dog. I know when Huey tore his toenail out, they were like, well, keep him from doing things. And I was like, he doesn't care if he's bleeding all over the floor. Like that's not a problem for him. So I had to ask for medicine, right? I had to ask for something to help sedate him just a little bit. So can you just talk about that? Yes. And I did go through it for about six months with Quinn where the only exercise she got was walking on a leash. Now she got other mental stimulation and some basic training, low impact training, but no running, no jumping. So when I talk about restricted activity, I do not mean crate rest. So I don't, there's only two conditions where I recommend really serious crate rest where the dog is in a crate or an X-pen unless it's out for a potty walk. And that's a dog with a fractured pelvis that is not amenable to surgery or the owner can't afford surgery or a dog with severe pain or neurological deficits from disc disease. And for some reason, the dog is not having surgery. So other than the two cases, for most of the soft tissue injuries that I see, restricted activity to me means no running, no jumping, no training, no playing with other dogs, no running up and down the stairs. I tell the owner, it's okay to walk your dog slowly up the stairs at night if they sleep in your room or down the stairs in the morning if you have to. Leash walk only so they can be out on a leash for five or 10 minutes several times a day sniffing around, but no or minimal trotting, no running.

그리고 집 안에 있을 때도 주의해야 합니다. 거실에서 TV를 보며 쉬고 있다면 반려견을 그 방 안에 머물게 해서 옆에 눕혀 두는 식으로 관리해야 하죠. 아니면 부엌에서 일을 할 때 반려견이 곁에 있게 할 수도 있습니다. 하지만 저는 사람들에게 리드줄로 반려견을 허리에 묶어두거나 방에서 나오지 못하게 하라고 말합니다. 왜냐하면 초인종이 울리면 모든 반려견이 집을 가로질러 문으로 전속력으로 달려갈 것이라는 걸 우리 모두 알고 있기 때문입니다. 아니면 한 마리가 창가에서 다람쥐를 보고 짖으면, 활동 제한을 해야 하는 반려견도 창가로 달려갈 것입니다. 그러니 그 부분은 주의해야 합니다. 하지만 활동 제한이 곧 크레이트 휴식(케이지에 가둬두기)을 의미하는 것은 아닙니다. 물론 활동량이 많은 반려견의 활동을 최소화하는 것은 어려운 일입니다. 그리고 저는 지금까지 활동 제한 때문에 죽은 반려견을 본 적이 없다고 말씀드리고 싶습니다. 또한 반려견의 활동을 제한해야 해서 죽은 보호자도 본 적이 없습니다. 모두가 생각보다 훨씬 덜 어렵게 이 과정을 잘 이겨냅니다. 트라조돈(trazodone)이라는 약물이 있는데, 아주 순한 진정제입니다. 사람들은 이렇게 생각하기 때문에 아주 좋은 약이죠. 나는 우리 강아지가 약 때문에 뻗어버리는 건 원치 않아. 하지만 트라조돈의 좋은 점은 적절한 용량을 투여하면 반려견을 약간 차분하게 만들어준다는 것입니다. 반려견을 조금 더 다루기 쉽게 해주고, 제한된 활동을 더 잘 받아들이게 합니다. 저는 퀸에게 그 약을 사용했습니다. 그래서 저도 그에 대한 경험이 있죠. 정말 좋은 약입니다. 만약 반려견을 더 진정시켜야 한다면 사용할 수 있는 다른 약물들도 있습니다. 하지만 굳이 그럴 필요까지는 없을 겁니다. 보호자들이 '오, 그건 불가능해요'라고 말하는 대다수의 반려견들, 아니 거의 모든 반려견들은 결국 해냅니다. 저는 할 수 없을 거예요라고 말하죠. 하지만 그들은 해냅니다. 그리고 반려견도 그냥 적응하게 됩니다. 반려견은 '아, 이제 이게 일상이구나'라고 생각하게 되는 거죠. 그리고 반려견이 정신적으로 활동할 수 있는 것들을 해준다면, 우리 모두 알다시피 생각하고 집중하게 되면 강아지들도 피곤해하기 때문입니다. 숲속을 달리는 것과는 완전히 같지는 않을지 몰라도, 분명 도움이 됩니다. 그냥 참고 견뎌내야 하죠. 꼭 해야 하는 일이니까요. 그렇군요. 전에 기준이 되는 영상을 확보하는 것의 가치나 그 개념에 대해 이야기 나눈 적이 있는 것으로 압니다. 하지만 이런 주제를 다룰 때마다 언급하면 참 좋은 내용이라고 생각해요. 그러니 반려견이 아직 건강할 때 나중에 혹시라도 다쳤을 경우를 대비해 미리 해두면 도움이 될 만한 것들이 무엇인지 좀 말씀해 주시겠어요? 우선, 사진이 확실히 도움이 됩니다. FENSI에서 피트니스 수업을 진행하는 강사들이 훨씬 많아졌어요. 저는 그들이 무엇을 가르치는지 알고 싶고 또 그들에게 배우는 점도 많아서 모든 수업을 다 듣습니다.
And you have to be careful when they're in the house if you're in a room that you can confine the dog to the room and you're all there and you're sitting on the couch watching TV, the dog can lay next to you. Or if you're in the kitchen working, they can be with you. But I tell people either tie the dog to your waist with a leash or plop off the room because we all know if the doorbell rings, all the dogs can be running full speed across the house to the door. Or if one dog's barking at the window at the squirrel, the dog that's supposed to be on restricted activity is going to run to the window. So you have to be careful about that. But restricted activity does not mean crate rest. Now, it is hard with high drive dogs to to minimize your activity. And I will say that I've never had a dog die from restricted activity. And I've never had an owner die from having to restrict their dog. They all manage to get through it with a lot less difficulty than they think it's going to be. There is a drug called trazodone, which is very mild sedative, which is a really nice drug because people think, I don't want my dog knocked out on drugs. But the nice thing about trazodone is when you get the right dose, it just takes the edge off. It makes the dog a little easier to handle and it makes the dog accept its restrictions a little better. And I use that with Quinn. So I do have experience with it. And it's a really good drug. And there are other drugs we can use if we need to sedate the dog more. But you shouldn't have to. I have to say the vast majority, if not all, of the dogs that the owners say, oh, it's impossible. I won't be able to do it. They do it. And the dog just gets used to it. The dog says, oh, well, I guess this is normal now. And if you can do things to work their mind, because we all know when they have to think and concentrate, it gets them tired. Same as, you know, maybe not quite the same as running in the woods, but it does help. And you just have to get through it. It has to be done. Fair enough. I know we've talked before about the value or kind of the idea of having some baseline video. But I do think it's a really good conversation to mention anytime we're talking about this stuff. So can you just talk a little bit about, you know, what folks should and could do now while their dog is presumably healthy that might be helpful should their dog be injured in the future? Well, for sure, photographs. And there's a lot more people teaching fitness classes for FENSI. And I take them all because I want to know what they're teaching and I learn from them.

그렇게 하면 나중에 강사님이 정말 훌륭하니까 이 수업을 추천한다고 말할 수 있거든요. 지금까지는 모두 다 훌륭했어요. 그런데 그들은 모두 반려견의 정상적인 자세에 대해 이야기합니다. 즉, 정상적으로 서 있을 때, 앉아 있을 때, 그리고 엎드려 있을 때의 자세 말이죠. 그러니 무엇이 정상적인 자세인지, 혹은 균형 잡힌 자세가 어떤 모습이어야 하는지를 배우고 반려견의 사진을 찍어두는 것이 중요하다고 생각합니다. 그리고 6개월이나 1년마다 사진을 찍어서 비교해보면 뭔가 이상이 있는지 알 수 있을 거예요. 만약 반려견의 등 라인이 항상 평평했는데 갑자기 허리 부분에 약간 굽은 모습이 보인다면, '음, 이건 예전엔 없던 건데'라고 생각할 수 있죠. 혹시 문제가 있는 건 아닐까 하고요. 또는 반려견이 뒷다리를 충분히 뒤로 빼서 중족골이 바닥과 수직을 이루는 아주 균형 잡힌 자세로 항상 서 있었다면 말이죠. 그런데 요즘은 '음, 세울 때마다 다리를 조금씩 앞으로 내미네'라는 걸 알아차릴 수도 있잖아요. 그렇다면 그게 정상은 아니라는 걸 알게 되는 거죠. 영상을 찍어두는 것도 분명 도움이 될 겁니다. 제가 좋아해서 수강생들에게 촬영해 보여달라고 하는 영상은 반려견이 일정하고 안정적인 속도로 속보로 걷는 모습입니다. 그리고 영상은 옆에서 촬영되어야 속보로 걷는 모습을 몇 걸음 이상 제대로 확인할 수 있습니다. 제가 속보를 좋아하는 이유는 대칭적인 걸음걸이기 때문입니다. 그래서 어디가 이상한지 파악하기가 더 쉽죠. 그다음에는 반려견이 저를 향해 속보로 오게 해서 앞모습을 확인하고, 반대로 멀어지게 해서 뒷모습을 확인합니다. 그러니 6개월이나 1년마다 그렇게 해보고 싶다면 좋은 방법입니다. 반복하지 않더라도, 반려견이 어리고 건강하며 상태가 좋을 때 지금 한 번이라도 해두면, 나중에 뭔가 이상하다고 생각될 때 비교할 수 있는 기준점이 생기는 셈이죠. 일부 수업에서 수강생들이 배우고 있는 또 다른 것은 반려견 스트레칭 방법입니다. 저는 매일 하지는 않지만, 어질리티 대회가 끝난 후에는 반려견의 어깨를 스트레칭해주고 모든 관절을 굽히고 펴주는 동작을 합니다. 그리고 고관절을 펴주기도 하고요. 장요근에 긴장을 주는 스트레칭을 해서 통증이 있는지 확인하거나 장요근을 촉진해보기도 합니다. 그런 다음 목 스트레칭을 합니다. 그래서 저는 제 반려견들에게 정상적인 스트레칭 범위가 어느 정도인지 알고 있습니다. 만약 스트레칭을 시도했을 때 오른쪽 어깨가 평소처럼 충분히 앞으로 나가지 않는 것을 반복적으로 발견하고, 개가 약간 아파하는 것 같다면 어딘가 문제가 있다는 것을 알 수 있죠. 이런 것들이 보호자가 할 수 있는 몇 가지 방법입니다. 또한 반려견 마사지 방법과 근육을 촉진하는 법을 배울 수도 있습니다. 제 반려견들은 어질리티 대회가 끝나면 항상 특정 부위에 약간의 통증을 느끼곤 합니다.
And then I can say, yes, I recommend this class because the instructor is fantastic. And so far, they've all been fantastic. But they all talk about what normal posture is for a dog. So what normal standing posture, sitting posture and posture when the dog is down. So I think it's important to learn what normal posture is or what balanced posture should look like and then take a photograph of your dog. And then if you take a photograph every six months or every year and compare them, you will know if something's off. So if your dog always has a flat top line and now they have a little roach in their lumbar area, you can say, hmm, that wasn't there before. I wonder if that's a problem. Or if your dog always has a nice balanced stand with the hind legs far enough back that the metatarsis is perpendicular to the floor. And now you notice, hmm, every time I get her into a stand, she moves his leg forward just a little bit. Then, you know, that's not normal. It would not hurt to have video. And the video that I like and that I ask my students to show me is with the dog trotting at a nice steady pace. And the video is taken from the side so I could see several good strides of trotting. And the reason I like trotting is it's a symmetrical gait. So it's easier to tell if something's off. And then I'll have them trot the dog towards me so I can see the front end and then trot the dog away. So if somebody wants to do that every six months or a year, then that's a good thing to do. And even if you don't repeat it, even if you just do it now when the dog is young and healthy and fit, then if you think something's off, then at least you have a baseline to compare it to. The other thing that students are learning in some of these classes is how to stretch your dog. So I have not frequently, it's not every day, but after agility trials, I'll just stretch my dog's shoulders and flex and extend all her joints. And I'll extend her hips. And I will do a stretch that stresses the iliopsoas muscle so that I know if there's any soreness in the iliopsoas muscle or I'll palpate the iliopsoas muscle. And then I'll do neck stretches. And so I know what normal stretching is for my dogs. So if I ever go to stretch her and her right shoulder doesn't go as far forward as it normally does, and I get that finding consistently and she seems a little sore, then I'll know that there's a problem there. So those are some things that owners can do. They can also learn how to massage their dogs and palpate the muscles. My dogs always are a little bit sore in certain areas after an agility trial.

And I know that if I just massage them and let them rest, then that soreness goes away and it's just not a problem. So you can't know what's abnormal until you know what's normal. And so you have to get to know what's normal for your dog. And this also gets the dog used to be handled. So if a veterinarian has to examine the dog, it's not like it's the first time they've ever had their shoulder extended is at the vet. Yeah. So part of the reason we're talking about all this stuff is you've got a new webinar on the calendar. What's the diagnosis happening May 9th at 3 p.m. Pacific time? Do you want to share just a little more on, you know, what you'll cover in the webinar and maybe who might want to join us? Right. So, I mean, anybody can join because I think it's important for anybody to know how to get a diagnosis on their dog's injury. And I just and we'll talk about this in the webinar. But I just want to say that hind leg lameness is not a diagnosis. Hind leg lameness is a clinical finding. It doesn't tell us why the dog is lame. If you see a rehab vet and they say, oh, you know, the back is sore. That's not a diagnosis. That's a clinical finding. We don't know why the dog's back is sore. A lot of times people will go to a chiropractor if they have a problem with their dog and the chiropractor will find a rib is out or this is out or and they adjust the dog. But that's not a diagnosis. So if you go to a chiropractor and then your dog is absolutely fine and you have no more problem, that's great. But a lot of times if you go to a chiropractor, either the dog does not get better or the dog is better for a day or two and then has a problem again. So the most important thing that we'll talk about in the webinar is why it's important to get a diagnosis and what a diagnosis is. We'll talk more about the different specialists that you can see for a diagnosis. And we'll talk about the certifications that some vets get in rehab that you could use that vet as for their special training. And what's the difference between a board certified specialist and somebody with a certification? And then I have two case histories. Both are students from last year's sports medicine class and both had dogs that did not get diagnosed for over a year with the problem. And when the dog finally got diagnosed and got proper treatment, both dogs got better. So we will walk through what they did. And both dogs were treated for over a year for clinical signs that sore back or or some mesolatrophy or whatever, whatever they were being treated for.

They were being treated for something that was not what the real problem was. And that's why the dogs weren't getting better. So people will learn why it's important to get a diagnosis and how to get an accurate diagnosis. And then once you have an accurate diagnosis, then you'll have a guide to the proper treatment to get the dog better. So kind of on that note, Sue, before I let you go. So if you get a diagnosis and you're treating it following a treatment plan, you know, is it reasonable to ask at that point? Kind of, you know, how soon should we begin seeing progress or something like that? And if those kind of guidelines aren't met, is it time to maybe think it's a misdiagnosis? Is there something in there? Yes. And that's really important because what I usually when I make a diagnosis of a soft tissue injury and I can usually narrow it down to the joint. I can't necessarily make a definitive diagnosis without radiographs or an ultrasound exam or some other tests. But I always give the owner the option of treating the dog conservatively for a couple of weeks or doing further diagnostics now. And a lot of them choose to do the conservative treatment. So I would work with Lynn, who was my physical therapist, and we would do the exam together. We would make diagnosis or a tentative diagnosis. And then she would see the dog once or twice a week for physical therapy, whatever that physical therapy should be. And I would recheck the dog in three to four weeks to see if they're getting better. So how do I know if they're getting better? The owner notices less lameness. They notice the dog seems more comfortable. And on my exam, I don't get pain or I don't get as much pain as I did when I palpated the area that was affected when I first saw the dog. So when I do a recheck in three or four weeks, if the dog is showing progress, they don't have to be 100% better. But if the dog's showing progress, then we continue with that treatment. If the dog is not getting better, then I tell the owner, we need to do further diagnostics because I might not have the right diagnosis or it might be something that needs more than conservative treatment. So if I'm treating a dog for an ileusose muscle strain, most of the dogs that have a grade one strain, which is the mildest form of injury, will show improvement in three to four weeks. They're not better in three to four weeks, but they're showing improvement. So then we just keep doing what we're doing. So we do physical therapy, pain control until I get no more pain on palpation of the muscle. And then we go from rehab to conditioning.

And after conditioning, we go to return to sport. So it's a whole process over several weeks to several months, depending on the injury. If I see the dog at three or four weeks and they still have a lot of pain, then I'm suspicious that they might have a more serious injury. So maybe a grade two muscle strain, whereas there is some tearing of the muscle fibers. And sometimes those don't get better without further treatments, advanced treatments like regenerative medicine, stem cells and platelet-rich plasma and things like that. So it depends on the dog and it depends on how they're doing. It depends on how much time and money the owner has. Sometimes doing advanced diagnostics and treatment, it's very expensive. Not sometimes. It is very expensive. We're talking about thousands of dollars. So we always have to balance that with, well, should we stay the course and continue doing conservative treatment for another three or four weeks, which is not wrong. So there's no wrong or right. There's no right answer or wrong answer or better or worse. It depends on the owner. It depends on the dog. It depends on where somebody lives. If you have to drive eight hours to go to a specialist, you might be less likely to do that than if you have to drive 30 minutes to see a specialist. But the most important thing is, and I have a slide in my webinar because we'll discuss this. In my opinion, and this is not in any book, but in my opinion, based on my training experience, if a dog's not getting better in a reasonable amount of time, and reasonable can vary, but I would say three to six weeks, either we have the wrong diagnosis or we're doing the wrong treatments. And I see a lot of my students that take my sports medicine class. They're dogs and also clients I used to see in my sports medicine practice. The dogs are being treated for one thing or another for months and sometimes years. And when I see the dog and we do an exam and we do further tests, they're being treated for the wrong thing. And that's why the dog is not getting better. So it's fine to try, quote unquote, conservative treatment for almost any injury. But if they're not getting better, we have to figure out why. Excellent. All right. Any final thoughts or key points you just want to leave listeners with? The only the reason I'm doing this webinar is I see posts on the FENSI alumni Facebook page all the time about people asking questions about their dog's medical issue. And and sometimes I contribute to the thread and sometimes I don't. But a lot of times they don't have a diagnosis.

So they're asking for advice on what to do with their dog when nobody really knows what's wrong with the dog.
